WHAT WE DO

NHTD - TBI

The Nursing Home Transition and Diversion (NHTD) Medicaid Waiver is a Home and Community Based Services (HCBS) program that helps individuals with disabilities and seniors transition from institutionalization back to their communities. The program is run by the New York State Department of Health (DOH) through Regional Resource Development Centers (RRDC) and Quality Management Specialists (QMS). The RRDC employs the Regional Resource Development Specialist (RRDS) and Nurse Evaluator (NE) for specific counties throughout the State.


Similarly, the HCBS/TBI waiver is administered by the DOH and implemented through the RRDC and RRDS specialists to help individuals with a traumatic brain injury (TBI) reintegrate into their communities. The waiver uses Medicaid funding to provide supplementary services and supports to assure the health and welfare of the individual in the community. Participants may choose to move out of nursing facilities or prevent institutionalization.


Service Coordination / Case Management


Our Service Coordinators/Case Managers offer day-to-day coordination and support to families and children. They assess the strengths and needs of family members and consumers, develop service plans, and work with behavioral health providers and community resources to implement them.
