What we offer

The New York State Department of Health (DOH) oversees two HCBS Medicaid waiver programs: Nursing Home Transition and Diversion (NHTD) and Traumatic Brain Injury (TBI). The NHTD waiver is administered by the DOH through contractual agreements with Regional Resource Development Centers (RRDC) and Quality Management Specialists (QMS). RRDCs employ Regional Resource Development Specialists (RRDS) and Nurse Evaluators (NE) to serve specific counties in the state. The NHTD waiver helps seniors and individuals with disabilities transition from nursing facilities or other institutions to the community, or prevent institutionalization through diversion. Waiver services are provided when informal supports, local, state, and federally funded services, and Medicaid State Plan services are insufficient to ensure the individual's health and welfare in the community.


The HCBS/TBI waiver is also administered by the DOH and implemented through RRDCs and RRDSs across the state. This waiver program assists individuals with TBI in achieving successful inclusion in the community. Participants can choose to move from nursing facilities to the community or prevent unnecessary institutionalization through diversion. The HCBS/TBI waiver services supplement informal supports, local, state, and federally funded services, as well as Medicaid State Plan services to ensure the individual's health and welfare in the community.


Service Coordination / Case Management

Our Service Coordinators/Case Managers provide families and children with day-to-day coordination and support. They are responsible for reviewing clinical reports, assessing the strengths and needs of family members and consumers, and developing a plan for service. Collaborating with existing behavioral health providers, as well as other community and local resources, traditional and non-traditional, our team assists families in implementing their service plan. Our Service Coordinators/Case Managers are fully trained in a variety of areas such as NHTD/TBI (Nursing Home Transition and Diversion/Traumatic Brain Injury), Medicaid Eligibility, OPWDD, and more. Whatever the need may be, we are here to help. From Food Stamp Assistance and HRA to Public Assistance, Medicaid Applications, Fair Hearing Services (as needed), Housing Assistance, or just guidance, our team is trained to assist families in all areas of need.
